Purdy completed 25 of 34 passes (73.5 percent) for 295 yards and a career-high five touchdown passes, posting a 126.8 passer rating in the 49ers' 48–27 road win over Indianapolis. It marked the third time since returning from a turf toe injury in Week 11 that the 49ers quarterback has recorded a passer rating of at least 126.8.
Purdy became the first 49ers quarterback to throw five touchdown passes in a game since Steve Young in Super Bowl XXIX and the first to accomplish the feat in the regular season since Joe Montana threw a franchise-record six touchdowns against the Atlanta Falcons on Oct. 14, 1990.
"He did awesome, man," head coach Kyle Shanahan said after the game. "Played really well. I got on him for a couple he missed, but it was pretty close to a perfect game, so he did a hell of a job."
The 49ers quarterback led the NFL in touchdown passes during Week 16 and topped the NFC in both passer rating and completion percentage.
"I think he's operating at an incredibly high level," tight end George Kittle said after the Week 16 win. "I thought he looked really good last week against Tennessee. I mean, he did a lot less running around today, too, than he did last week, so he has a bunch of different elements to his game that allow him to be a really good quarterback and play at a super high level for us."
This marks the second NFC Player of the Week award of Purdy's NFL career. He joins Pro Football Hall of Famers Steve Young (11 awards) and Joe Montana (eight), along with Jeff Garcia (two), as the only quarterbacks in 49ers history to earn multiple Player of the Week honors.
Purdy is also the only former Iowa State quarterback to receive multiple NFC Offensive Player of the Week awards.
